At least those three are missing: libgcc_s_dw2-1.dll, libstdc++-6.dll, zlib1.dll.

Updated the zip. Mind giving it another check? Apparently those libraries had been added to my PATH without my recollection!

Ah. Apparently there's platform specific libraries from Qt that need to be included with the application in a "platforms" folder with the exe. Unfortunately I did not find that info on Qt's site. (of course not!)

Should work now; dug out an old windows laptop that did not have a bunch of dev libraries installed and it launches: http://ubuntuone.com/7aGe9Es2IJYHX7lORkW84l
